Maxim government services is seeking a full time Physical Therapist (PT) for services in the Community-based Outpatient Clinic (CBOC) in Santa Rosa functions as a member of a multidisciplinary treatment team.

He/She will perform physical therapy services in response to provider generated referrals in accordance with Rehabilitation Section policies and procedures and VA regulations governing the practice of physical therapy. Patients referred to the clinic present with a large variety of diagnoses including orthopedic, neurologic, and medical. PT will demonstrate competence in providing physical therapy services to this clinically diverse patient population with an emphasis in the area of orthopedics. This person is responsible for clinical and select administrative aspects of the CBOC PT clinic

PLACE OF PERFORMANCE: Santa Rosa CBOC Physical Therapy Clinic at 3841 Brickway Blvd, Santa Rosa, CA 95403

• License: Graduation from an accredited school of Physical Therapy and a current and unrestricted PT license to practice.
• Board Certification: not required
• Technical Proficiency: must have at least two years of recent experience in an outpatient clinic
• (within last 3 years) serving a patient population with orthopedic/medical/neurological diagnoses.
• Must be BLS certified.


Work Schedule: Monday through Friday from 8:00am-4:30 p.m.
Federal Holidays: clinic is closed, services not required


Contractor is responsible for these clinical tasks:

a) Demonstrates ability to interpret provider prescription and referral. Reviews and screens patient’s medical record for appropriateness of PT intervention, noting any potential risks or contraindications to treatment and to assist in guiding the evaluation process.
b) Performs comprehensive evaluations using standard and nonstandard methods. Evaluations are conducted independently and results are documented according to departmental and Medical Center standards. Interprets clinical signs and symptoms and makes appropriate referrals for further medical attention as indicated.
c) Addresses rehabilitation potential, behavior, motor skills and psychosocial development at an age appropriate and population specific level. Identifies barriers to learning and strategies to overcome them.
d) Independently develops appropriate treatment plans and goals; performs the full range of PT treatment procedures, modifying the treatment according to the patient’s response to PT intervention.
e) Recognizes adverse reactions to treatment and initiates effective action immediately. Modifies treatment plan according to patient response to best meet established goals. Documents progression and/or modifications according to Departmental and Medical Center Standards.
f) Regularly discusses with physicians and other team members patient performance in PT, current status, and continuing goals. Works effectively with treatment team members and other health care providers in coordinating the patient’s total care. Attends rounds and case conferences as requested
g) Ensures appropriate patient referrals and discharge planning in concert with the multidisciplinary treatment team. Makes certain that patient has been assessed for appropriate DME (durable medical equipment) and that such equipment has been ordered and issued.
h) Measures, fits and instructs patients for complex adaptive equipment and assistive devices. Provides documentation that justifies medical need for all adaptive equipment and assistive device requests.
i) Triage consults and generates non VA care referrals when appropriate.
j) Adheres to the principles of good customer service, including but not limited to: awareness of the importance of all internal and external customers, respect for patient confidentiality, and courteous and respectful communications with all customers.


Contractor is responsible for these administrative tasks:

a) Attends departmental staff meetings.
b) Therapist will follow scheduling guidelines and mandates, and collaborate with scheduling staff to facilitate maximal utilization of clinic slots.
c) Helps to maintain PIP inventory in conjunction with the Prosthetics Department.
d) Maintains environment of care in compliance with safety and infection control guidelines.

e) Records and submits in a timely manner productivity and workload information, including encounter information. Submits other data and reports as requested by Supervisor.
